Name: I know you got this. Who are you in the game.

Age: As long as if your not old enough to fall into the grave in a day.

Gender: Male? Female?

Appearance: What does your character look like and wear? You can use pictures or use words to explain your character. Using both is best.

Skills: What are you good at and what do suck at. It also can be for personality.

Weapons: This is where you list 1 gun and 1 other weapon. You will get more weapons during the game.

Other Items: List anything else of note here, such as supplies, memoirs, etc.
